1. How many defendants are to be served?

2. If more than one (1) is the Address the same or different?

3. In what Cities/Town/County are they to be served?

4. Is the address a residence, business, Government or Military Base?

5. How fast do you need the server? STANDARD, RUSH or EMERGENCY = Stop what you’re doing and server now.

6. In what manner must service be affected?

A: Personal-in-hand.

B: A person of suitable age.

C: Affix and mail.

D: Any of the above.

7. By what date must documents be served?

8. Is Court filing required?
